Health care professionals may register by filling out forms on our web site giving all of the information above plus fax number, board certifications currently held, number of years in practice, the countries in which they are licensed, and the states in which they are licensed (if licensed in the United States). In addition, we will ask healthcare professionals to complete a “Get Started Profile” that will provide us with information on their use and experience with various aesthetic injectable products. Providing the information described above is optional, however, it is required for registration and the benefits that registration provides.

In addition to providing the foregoing information, our web site gives information about how you may contact us for further information about our products. We provide our phone numbers, street address and e-mail contacts.
